The Department of Public Welfare supports programs for individuals with disabilities throughout the state with a focus on individualized care and benefits programs designed to meet the specific needs of each person. From assisted living and personal care homes, to individual supports plans for community services, to Medical Assistance, DPW seeks to meet the needs of individuals with disabilities throughout the community. 2012 Permanent Supportive Housing Award
This annual award recognizes successful initiatives to provide affordable housing in Pennsylvania, with an emphasis this year on housing for people with disabilities. The award is given to both an individual and to an organization that has increased permanent supportive housing opportunities for people with disabilities and has shown a willingness to take risks and experiment with new models, approaches, and strategies. If you know of an individual or an organization that should be recognized for their efforts, please use the nomination form indicated below. Nominations are being accepted until Aug. 17, 2012. The winners of this year’s award will be announced at the Housing Alliance of Pennsylvania’s Annual Conference in December.
